### Introduction
Welcome to my reflection on interprofessional collaboration within a healthcare setting. In this video, I will discuss a specific interprofessional collaborative project that I experienced, examine its successful and unsuccessful aspects, and suggest strategies for improving collaboration based on best practices from the literature.
### Reflection on an Interdisciplinary Collaboration Experience
During my time at a hospital, I participated in an interprofessional collaboration aimed at improving patient discharge processes. The team comprised nurses, physicians, social workers, and pharmacists. Our goal was to streamline discharge procedures to reduce patient readmission rates and enhance patient satisfaction.
