Novice – 0 points The goal is general and does not address areas of weakness in clinical practice. The goal displays no specificity, is general and vague. 8 points
Measurable Goals may have several short-term and on-going measurements so that you can see how you are doing in your aim to achieve your goal. A non-measurable goal: I want to learn how to sing. A measurable goal: I want to learn 5 arias by Friday, November 12th Exemplary – 6-8 points Measures have been identified that will demonstrate learning, results, and/or progress as related to the described goal. Distinguished – 4-5 points A method of measuring has been identified but it does not directly link to or connect with the described go. Developing – 1-3 points The method listed for measurement is rather general and does not directly link to or connect with the described goal. Novice – 0 points Not at all measurable – no method of measurement indicated. 8 points

Distinguished – 4-5 points The goal set forth is realistic; however, the achievability may not require much challenge, or the achievability may be incredibly challenging to achieve within the term. The process is mentioned but lacks clarity. Developing – 1-3 points The goal is realistic, however the time frame does not support success. There is no process that supports achievability. Novice – 0 points The goal is not at all within reason and cannot be achieved. 8 points
Relevant Realistic goals should have you push you, but not break you! Exemplary – 6-8 points The goal has a strong connection to the course objectives, the student’s history, current interests, and/or demonstrated abilities. Distinguished – 4-5 points The goal has some connection to the course objectives, the student’s history, current interests and/or demonstrated abilities. To help you create your assignment on SMART goals for your clinical experience as a Psychiatric Mental Health Nurse Practitioner (PMHNP), here are five professional goals, each outlined according to the SMART criteria: Specific, Measurable, Achievable, Relevant, and Timely. Each goal summary is written to meet the requirements of at least 100 words, and I’ll include guidance on how to format this into a PowerPoint presentation.

### 1. **Goal: Improve Diagnostic Proficiency in Mood and Anxiety Disorders**

**Specific:** I aim to enhance my ability to accurately diagnose mood and anxiety disorders in patients during clinical assessments. This includes differentiating between similar conditions such as major depressive disorder and generalized anxiety disorder. The focus will be on understanding patient history, presenting symptoms, and appropriate use of diagnostic tools.

**Measurable:** I will measure progress by achieving at least a 90% accuracy rate in diagnosing mood and anxiety disorders, as confirmed by my supervising clinician, based on initial assessments and subsequent treatment outcomes by the end of the term.

**Achievable:** This goal is achievable by engaging in regular supervision, participating in relevant case discussions, and reviewing current diagnostic criteria from the DSM-5. I will also practice differential diagnosis in a simulated environment.

**Relevant:** Accurate diagnosis is a fundamental skill for a PMHNP, directly impacting patient care and treatment outcomes. This goal aligns with the clinical course objectives focusing on diagnostic skills.

**Timely:** The goal will be achieved by the end of the clinical term, with a review of diagnostic accuracy rates conducted every month to track progress.

### 2. **Goal: Master Medication Management for Bipolar Disorder**

**Specific:** I want to master medication management for patients with bipolar disorder, focusing on understanding pharmacokinetics, side effect profiles, and monitoring strategies for mood stabilizers such as lithium and anticonvulsants.

**Measurable:** Success will be measured by developing individualized medication management plans for at least 5 patients with bipolar disorder, achieving a 90% adherence rate to these plans by patients and maintaining stable mood states, as tracked through patient records and follow-up assessments.

**Achievable:** The goal is achievable with dedicated study, support from my supervising clinician, and applying knowledge from pharmacology coursework and clinical guidelines. I will participate in medication management workshops and case reviews to enhance my understanding.

**Relevant:** Effective medication management is essential for managing bipolar disorder and preventing relapse, making this goal crucial for clinical competence and patient care quality as a PMHNP.

**Timely:** This goal will be completed by the end of the clinical term, with bi-weekly assessments to adjust treatment plans as needed and evaluate adherence and patient outcomes.

### 3. **Goal: Develop Advanced Skills in Cognitive Behavioral Therapy (CBT)**

**Specific:** My goal is to develop advanced skills in delivering Cognitive Behavioral Therapy (CBT) for patients with depression and anxiety. This includes mastering specific techniques such as cognitive restructuring, behavioral activation, and exposure therapy.

**Measurable:** Progress will be measured by successfully conducting at least 10 CBT sessions, receiving feedback from patients and my clinical supervisor, and achieving a competency rating of at least 85% by the end of the term.

**Achievable:** This goal is attainable through active participation in CBT training sessions, supervision, and regular self-reflection on session effectiveness. I will also review CBT literature and apply techniques in practice sessions.

**Relevant:** Developing CBT skills is vital for PMHNPs, particularly in managing patients with depression and anxiety, aligning with the clinical course’s objectives on therapeutic interventions.

**Timely:** I aim to achieve this goal by the end of the clinical term, with progress reviews after every three sessions to ensure consistent improvement.

### 4. **Goal: Enhance Interprofessional Collaboration and Communication Skills**
