- Once your Faculty Advisor has approved the documents above that were submitted in the course room, set up a meeting in Teams with your DNP mentor, any other site stakeholders, your Faculty Advisor and any other stakeholders. During this meeting you will present your final Action Plan using the supplied PP template.
- Create a zip file with your presentation, project materials, and the documents listed above and submit to Task Stream for committee review and approval
- Complete your Ethics Pledge online
- Note: all of Phase 2 must be completed for a satisfactory grade for the second semester of N8702
Phase 3: Implement

Practice Problem The title for the planned DNP project is ‘Tangible Virtual Treatment’ and the setting will be virtual consult MD, which is in private practice. The project will take place in Norcross, Georgia, USA, and will target the practice gap in my organization of the undermining of patient physical monitoring before the administration of medication for their various mental health conditions. This quality improvement practice gap is not only an issue in my organization but also in other settings in healthcare. Physical monitoring of vital signs and other areas such as weight and blood level for illicit drugs is critical for mental health care for patients in the present and the future as it guides the characteristics of the health condition and the patient’s body’s ability to deal with the resulting stress on the wellbeing of their various physiological components. Goals The proposed type of project is a quality improvement initiative, where the initiative must be in line with the practice gap in the healthcare organization. An improvement initiative seeks to improve the identified practice gaps in an organization by implementing the related interventions. For instance, a quality improvement initiative for medication administration without consideration for patient monitoring is educating the staff within the organizational setting on the same. Such education reminds care providers of the basics of patient treatment that they may have forgotten or undermined during their practice and promotes -evidence-based practice in healthcare organizations while closing practice gaps. |

LONG-TERM GOAL | The objectives will target care providers providing mental health services in the organization. Feedback will be gained from administrative records on checking vital signs and physical patient monitoring. The objectives are attainable as they only require the data obtained from providers in my organization. This quality improvement will promote positive outcomes for patients dealing with mental health issues. Monitoring the data from medication management in the organization will require about 3 months as such time is enough to track the critical changes.

DNP Project Summary
The title for the planned DNP project is ‘Tangible Virtual Treatment’ and the setting will be at the private practice Virtual Consult MD in Norcross, Georgia, USA. The private practice offers telepsychiatry to individuals across the lifespan. Physical monitoring before recommending and prescribing medication for various mental health conditions is not required. This quality improvement practice gap is an issue in my organization. For instance, research indicates that less than 50% of patients on prescription drugs in mental care settings undergo critical monitoring when they seek care in England (Ayre, Lewis, and Keers, 2023). Physical monitoring of vital signs and other areas such as weight and blood level for illicit drugs is critical for mental health patients. Furthermore, it guides the characteristics of the health condition and the patient’s body’s ability to acclimate to the resulting stress on the well-being of their various physiological components (Sapra, Malik, and Bhandari, 2020).
The proposed type of project is a quality improvement initiative. This initiative is in line with the practice gap in my healthcare organization. A quality improvement initiative seeks to improve the identified practice gaps in an organization by implementing the related interventions. For instance, a quality improvement initiative for medication administration without consideration for patient monitoring will also educate staff within the organizational setting. Therefore, reiterating the basics tenants of patient care promotes -evidence-based practice in healthcare organizations while closing practice gaps.
